Long Term Disabled Parking Permits (Blue)
Long Term Disabled Parking Permits (blue) are available to any individual with permanent mobility problems or certified legally blind. Applicants must submit a completed application Form HSMV 83039. The disability must be certified by a licensed chiropractor, physician, osteopathic physician, podiatrist, advanced registered nurse practitioner, physician assistant, or the Adjudication Office of the Veterans Administration. Only original, computer generated, laser printed or facsimile (stamped) signatures are acceptable on applications. Form must be dated within the past 12 months. The applicant must have a valid Florida Driver License or Florida Identification Card. Applications submitted with a 100% Disability Rating Card or Form 27-333 within the last 12 months issued by the Veterans Administration are not required to have Form HSMV 83039 completed by a certifying authority. One additional permit may be issued to persons who certify they are frequent travelers or are certified as quadriplegic. Permits are valid for four years. There is no fee for issuance of a blue Long Term Disabled Parking Permit.
Renewal Information for Long Term Disabled Parking Permits (Blue)
Prior to expiration, a renewal notice will be mailed from the Florida Division of Motor Vehicles for blue Long Term Disabled Parking Permits. Effective October 1, 2012, the disability must be recertified on a Form HSMV 83039 at each renewal by a licensed chiropractor, physician, osteopathic physician, podiatrist, advanced registered nurse practitioner, physician assistant, or the Adjudication Office of the Veterans Administration. Only original, computer generated, laser printed or facsimile (stamped) signatures are acceptable on applications. Form must be dated within the past 12 months. Applications submitted with a 100% Disability Rating Card or Form 27-333 within the last 12 months issued by the Veterans Administration are not required to have Form HSMV 83039 completed by a certifying authority. There is no fee for renewal of a blue Long Term Disabled Parking Permit.
Replacement Information for Long Term Disabled Parking Permits (Blue)
Effective July 1, 2012, to replace a lost or stolen disabled person parking permit, the disability must be recertified on Form HSMV 83039 by a licensed chiropractor, physician, osteopathic physician, podiatrist, advanced registered nurse practitioner, physician assistant, or the Adjudication Office of the Veterans Administration. Only original, computer generated, laser printed or facsimile (stamped) signatures are acceptable on applications. Form must be dated within the past 12 months. Applications submitted with a 100% Disability Rating Card or Form 27-333 within the last 12 months issued by the Veterans Administration are not required to have Form HSMV 83039 completed by a certifying authority. There is no fee for replacement of a blue Long Term Disabled Parking Permit.

Temporary Disabled Parking Permits (Red)
Temporary Disabled Parking Permits (red) are available to any individual with a temporary disability. Applicants must submit a completed application Form HSMV 83039. The disability must be certified by a licensed chiropractor, physician, osteopathic physician, podiatrist, advanced registered nurse practitioner, physician assistant, or the Adjudication Office of the Veterans Administration. Only original, computer generated, laser printed or facsimile (stamped) signatures are acceptable on applications. Form must be dated within the past 12 months. The applicant must have a valid Florida or Out of State Driver License or Identification Card. Permits can be valid for up to six months, as determined by the signing physician. There is a $15.00 fee for issuance of a red Temporary Disabled Parking Permit. There is no fee for replacement or an additional red Temporary Disabled Parking Permit if within a 12 month period from the date of the prior fee payment.
